The Family Support Specialist provides services to families to improve their lives based on their specific goals within the PI program. Social services include an evaluation of the families’ strengths and needs, assisting with supportive programs (such as SNAP, TANF, or affordable housing, etc.), and provides referrals to education and job placement programs that assist families in need. The Family Support Specialist assists families with understanding and implementing the Individual Family Service Plan (IFSP) to encourage and promote their child’s overall development. Also, to include the parents achievement of self-sufficiency, and positive developmental outcomes for all children. FSS will assist with recruitment efforts for the ISBE: PI program to ensure families with children birth-to-five, as well as pregnant women, are informed about Chicago Child Care Society’s home-based program.
Performs the functions of this position with a strong understanding of the ISBE: PI & BabyTalk Performance Standards; and demonstrates and assures professional best practices.
Enroll all birth-to-three children/families into the ISBE: PI home-based program.
Enter all ISBE: PI family assessments and family goals, and health history information into the NewOrg system at time of enrollment.
Review and follow-up, on the goals and progress of the families enrolled in PI-home visiting to support Family Advocates in their work with families.
Monitors and tracks all PI parent involvement which also includes monthly visits (to ensure ongoing participation), and social service activity, and follow-up with parents whenever there is any inconsistency presented.
Assist Family Advocates with monthly parent/child group by selecting/securing locations, assist with ensuring food is provided, and additional as needed.
Assist Family Advocates with caseload if a position becomes vacant or an staff member takes FMLA.
Assist Program Supervisor with locating HS/PFA center-based programs to refer 2.6month-olds to prior to them reaching their third birthday.
Collaborates with Family Advocates to ensure appropriate transition from home-based services to center-based programming is seamless and family needs are met.
Collaborates with Program Assistant to coordinate recruitment efforts and locations on a monthly basis to inform the boarder community about the PI- home visiting program and ensure full enrollment with a waitlist.
Maintains regular contact with families to keep abreast of needs and interest in the program, through additional home visits (when necessary), phone calls, and written communication.
Establishes and develops relationships with social service agencies to create partnerships for referrals and recruitment.
Ensures files and documentation are complete, accurate, and maintained confidentially for all enrolled children and families. In addition, conduct monthly random file audits to ensure all documentation is noted and in the files.
Participates in bi-weekly reflective supervision with Program Supervisor.
Participates in monthly reflective practice meetings with the Mental Health Consultant (MHC). Attend department staff meetings and all-staff (agency) meetings, and pre- and in-service training/conference sessions as scheduled.
Responds to agency needs by performing assigned tasks which may not fall within above description. If such tasks are not of a temporary nature, they should be added to this list of designated job responsibilities.
Job: Full Time, Exempt Position
A minimum of a Bachelors degree in Social Work, Human Services, or related degree from an accredited school.
Minimum of three-five years’ experience in Early Childhood Education and working with families, which includes direct service.
Have experience in collaborating with community partners and establishing working relationships.
